Healthcare Advocacy
Nurse Advocacy Through Hospice & End of Life Support, Assisted Living & Memory Care Transitions
-
Navigating a loved one’s changing care needs can feel overwhelming—especially when decisions need to be made about staying at home, transitioning to assisted living or memory care, or entering hospice care.
As a Registered Nurse Healthcare Advocate, I help individuals and families make these decisions with clarity, confidence, and clinical insight. My role is to provide guidance, education, and steady support so families feel informed and supported—not rushed or alone—during some of life’s most difficult transitions.
Supporting Care Decisions at Every Stage
Care needs often change over time. I help families understand and navigate each stage, including:
Centering home as the preferred setting whenever safe, while helping families put the right care supports in place to maintain safety, dignity, and independence
Providing education and support when considering assisted living or memory care communities
Helping families know what to look for, what questions to ask, and how to compare care options before making a decision
Supporting transitions into assisted living or memory care with a focus on safety, quality of life, and individual needs
Communicating with physicians, care teams, and facility staff to ensure continuity of care
Assisting with care coordination and next-step planning during transitions
Providing advocacy and guidance during hospice care, serious illness, and end-of-life decision-making
Supporting families in understanding advance care plans and honoring known wishes
A Thoughtful, Guided Approach to Care
Whenever possible, I support families in exploring home-based care options that promote comfort, dignity, and independence. When additional support becomes necessary, I help guide families through assisted living or memory care decisions so they can choose a setting that truly meets their loved one’s needs—not just the first available option.
I’ve seen how quickly families can be placed in the position of touring communities, reviewing contracts, and making major care decisions without knowing what questions to ask or what details matter most. My goal is to provide clarity and ensure decisions are made with understanding and confidence.
Compassionate Support When It Matters Most
For families facing serious illness, hospice care, or end-of-life transitions, I provide calm, steady support during an emotionally difficult time. This includes help communicating with healthcare providers, understanding care options, and ensuring that decisions align with the individual’s values and wishes.
Whether someone is planning ahead, transitioning care, or navigating end-of-life decisions, my focus remains the same: to provide knowledgeable, compassionate guidance so families never have to navigate these moments alone.
