As your appointed Medical Power of Attorney (MPOA), I may serve as your healthcare decision-maker if you become unable to make or communicate your own medical decisions. My role is to understand your values, wishes, and advance care planning documents and advocate for care that is consistent with them.
Depending on your individual circumstances and the scope of our agreement, my role may involve:
Serving as your legally appointed healthcare agent and medical decision-maker
Communicating with physicians, specialists, hospitals, hospice providers, long-term care communities, and other members of your care team
Reviewing and helping interpret medical information so that healthcare decisions can be made thoughtfully and with a clear understanding of your options
Communicating and coordinating with your Financial Power of Attorney, care managers, attorneys, and other professionals when healthcare decisions intersect with financial, legal, or care-planning considerations
Supporting healthcare transitions, which may include home care, rehabilitation, assisted living, skilled nursing, hospice, or end-of-life care
Advocating for your documented wishes, values, preferences, and quality-of-life goals
Providing a calm, knowledgeable presence during serious illness, changing health circumstances, or complex medical decisions
Helping maintain communication, continuity, and appropriate oversight as your healthcare needs evolve
Where appropriate and authorized by applicable documents, helping communicate and support the care needs of those you leave behind
